10/28/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA taught Ms. Broadnax how to use self-monitoring and developing affirmations. QMHPA encouraged Ms. Broadnax to use the techniques taught to help her reduce stress such as exercising in her home or outside of her home, walking away from situations that may be conflicting, and how to use positive self -talk. QMHPA taught Ms. Broadnax several social skills such as positive body language, moderate voice tones, self -monitoring and good problem- solving skills. QMHPA used modeling and coaching to help Ms. Broadnax process how to use her communicative skills respectfully. QMHPA taught Ms. Broadnax several skills for independent living such as improving her personal and social adjustments and decreasing dependence on others. QMHPA used role playing exercises to help Ms. Broadnax increase her Interpersonal skills such as communicating, problem solving and being consistent. Response: Ms. Broadnax appeared to be receptive to QMHPA’s interventions and services that were provided to her. Ms. Broadnax has agreed, not yell, bully, threaten or disrespect anyone. Ms. Broadnax was commended for avoiding outbursts of anger. Ms. Broadnax was able to identify how self -monitoring has helped process the importance of using her voice tones learn how to appropriately connect with others. Ms. Broadnax shared that she is learning how to communicate so that she can get along with her family and her friends. Ms. Broadnax admitted that her struggle is her aggressiveness. Ms. Broadnax was supported for identifying her areas of weakness and where she needs improvement. Ms. Broadnax shared that she still has no patience when learning how to use her relaxation skills. Ms. Broadnax has agreed to practice the techniques daily and then use those techniques she feels work better for her situations. Ms. Broadnax agreed to practice 10-15 minutes each day along with she has journaling. Ms. Broadnax admitted that she has not use active listening skills recently and feels it was a good idea to start. Ms. Broadnax was praised for being able to make appropriate adjustments during the session. Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
